Information on how to Join FREMAP

We provide you with the information and documentation you need to become a member of FREMAP.

Are you a Newly Established company?

  • Obtain and complete form TA.6 of the General Treasury of the Social Security (Form to register with the Social Security System).   • Complete box 7.1 on form TA.6 (Institution for Work-related Injury and Occupational Disease) with following information:
    • Name field: FREMAP (*)
    • Number field: 061 (*)

    (*) If you choose our forms, this information will appear by default to make it easier to complete the form.

    Should you want to choose the option to receive the benefit for Temporary Incapacity due to Non Work-related Injuries and Diseases with the mutual society, check the option 'Mutual Society' in box 7.2 (Institution to cover temporary incapacity due to non work-related injuries and diseases).

  • Submit form TA.6 together with the rest of the required documentation to the Offices of the General Treasury of the Social Security This link will open in a pop-up window.

Are you a company that is a member of another mutual society?

  • Ask to cancel your membership with your current Mutual Society at least one month before your membership expires, through:
  • Cancellation request letter, expressly identifying the signatory in the company: name, national ID number, and position.
  • According to the First Transitory Provision of RD 1622/2011, the company must obtain the non-binding report from the workers' committee or staff representatives, unless such bodies do not exist:
    • Report from the Workers' Committee or Staff Representatives (if there is a committee).
    • Certification that there is no worker representation (if there is none).
  • Fill in the application to join FREMAP with your company's data.
  • Obtain a Certificate of cancellation from your previous Mutual Society.
  • Deliver the following documentation to the Social Security General Treasury in duplicate, leaving one of the copies stamped by the Treasury in the possession of the company:
    • Cancellation certificate issued by the previous mutual society that was covering the contingencies.
    • Proposal to Join FREMAP signed and stamped by the company.
    • Some delegations of the aforementioned Body could request a photocopy of the DNI or power of attorney of the signatory to the request to join.

    Once the procedure is completed, the change will be effective starting on the day after the date specified in the cancellation certificate.

Are you a company that is a member of INSS?

  • Request the cancellation from the INSS, presenting the cancellation request document Social Security Delegation, at least one month before the expiry date of the association agreement.
  • Fill the application to join FREMAP.
    • If you also wish to have FREMAP cover for the Economic Benefit for Common Contingencies, you must fill in the Annexe to the membership document.
  • Submit the FREMAP Association Proposal together with the previously submitted cancellation document in the Social Security Institute.
  • Send a copy of the documentation submitted to the Social Security Treasury to your nearest FREMAP office.

Once the procedure has been completed, the change will be effective starting on the day after the date specified in the cancellation certificate.
