In order to complete your social security registration and be able to subscribe FREMAP's coverage for financial compensation for temporary disability due to common illnesses, work-related accidents and illness coverage and coverage for termination of activity, you must fill in form TA.0521 to register for the Special Regime for Self-employed Workers, specifying your mutual insurance company (FREMAP 061).

Once this documentation has been sealed at a Provincial Treasury Office
send your application or take it to your nearest FREMAP office. Centre Network
- Complete the cancellation request with the other Mutual Society.
- Submit, or send by any reliable means, two copies of your application to cancel your policy with your current mutual insurance company before 1 October each year. The change will be effective starting on 1 January of the following year.
- Send a subscription application letter to any FREMAP centre before 1 October. Said letter must be accompanied by:
- A sealed copy of the document requesting that your policy be cancelled, or a cancellation certificate if your current mutual insurance company has sent you one following such a request.
